If the clan is on StarLadder, log into your StarLadder account (or create one). Open the clan page on SL and click "Leave a request". The clan leader will see your request and decide whether or not you're accepted.
If the clan is on Forum, open their topic and post "I want to join this clan". If they have an application form (which usually is in the main (#1) post), fill it. Again, the leader will see your request and decide if you're accepted or not.
